Enrollment Process
Once your family has decided to join the U.S. Naval Sea Cadet Corps, you’ll be invited to enroll. Enrollment requires submitting a completed application packet along with the enrollment fees. All required forms can be downloaded from the links below. Every form is required unless otherwise noted.
Required Forms:
- Cadet Application (NSCADM001, Pages 1–2) – Signed in two places by a parent or guardian.
- Report of Medical History (NSCADM001, Pages 3–4) – Signed by both a parent/guardian and a physician.
- Report of Medical Examination (NSCADM001, Pages 5–6) – Completed, signed, and stamped by a physician.
- Medical History Supplemental (NSCADM001, Pages 7–8) – Required if the cadet takes any prescription or over-the-counter medications. Mark “N/A” if not applicable.
- Request for Accommodation (NSCADM001, Pages 9–10) – Required only if a medical condition limits full participation in the program. Signed by a parent/guardian; mark “N/A” if not applicable.
- Parental Support Agreement (NSCADM001, Page 11) – Acknowledges that NSCC leadership is made up of adult volunteers and outlines volunteer opportunities for parents and guardians.
- Immunization record – copy signed and stamped by your physician
- Health insurance card – copy of both sides needed
- Copy of most recent report card
